Activity Verification Form

Can be used for all socials, joint events, events with Greeks, etc. Submit one for each activity performed during the year with your annual chapter report.


Community Service Verification Form

Used for recording community service. Submit one for each community service event performed during the year with your annual chapter report.


Philanthropy Financial Form

Used for recording amount of money raised for philanthropies. Submit one for each philanthropy event performed during the year with your annual chapter report (if applicable).


Rush Workshop Form

Used for recording rush workshop material and attendance. Submit one for each rush workshop held during the year with your annual chapter report.


Rush Data Collection Form

Used to record rush activities and no. of attendees for each rush event. Submit one for each rush even held during the year with your annual chapter report.


Candidate Member Data Template

Used to collect information on each candidate. Submit to your regional field director for each new pledge class within 2 weeks following Induction.


Initiation Checklist

For reference only. Reminds membership educators what needs to be sent to their regional field director. Should be used with each new candidate class.


Senior Exit Survey

For graduating Seniors to complete during their last school term. Submit to the VP of Collegiate Affairs once completed.


New Alumna Data Form

Used to collect data for each new alumna member. Submit to the VP of Collegiate Affairs and the National Database Manager for new alumna prior to end of each academic term.


National Awards Form

Form Includes President's Commendation, Lifetime Achievement, Chapter Advisor and Alumnae Officer nomination forms. Submit form with your annual chapter report.


LEAP Workshop Request Form

This form is to help in requesting a LEAP workshop for your chapter. The form will help us to better understand your needs for leadership education and in turn provide you with better workshops in the future.
